How to apply 如何申请

Obtain an ESCS Application Kit from the Australian Acupuncture and Chinese Medicine Association Ltd.

 

The Application Kit includes:

-          Applicant Information Sheet

-          Application Form and Declaration

-          DEWHA Wildlife Booklet

-     DEWHA Fact Sheet- CITES

-     DEWHA Fact Sheet- Wildlife Permits 

-          Certification Examination

-         Anonymous Questionnaire

       Checklist

 

These documents (with the exception of the DEWHA materials) are can also be obtained in Chinese.  Please clicks the relevant links in the Chinese text below.  A hard copy of the Chinese versions of the DEWHA Wildlife booklet can be obtained from DEWHA or AACMA.

 

All sections on the Application Form should be completed.  Applicants must read and sign the Declaration Section on the Application Form in front of a witness and get the witness to sign and complete their details.

 

The Declaration Section has the following components:

-          authority for AACMA to make enquiries as it deems relevant about the information/documents supplied with the application from third parties, including but not limited to institution(s) or organisations listed on the form or in any attached documents;

-          agreement to be bound by the terms and conditions of certification under the scheme as outlined in the Applicant Information Sheet;

-          agreement to receive information and updates about the legislative framework, the Environment Protection and Biodiversity Conservation Act 1999 and the requirements for legal international wildlife trade;

-          declaration that the person/business will trade only in legally acquired wildlife parts and products and that medicines and other products derived from the illegal trade in endangered plants and animals will not be supplied by the business or from the premises;

-          agreement to be subject to the Disciplinary Committee (or its successor) of the Australian Acupuncture and Chinese Medicine Association Ltd in relation to complaints under the scheme;

-          agreement to the release of personal information to the Department of the Environment, Water, Heritage and the Arts about certification issued under the scheme as well as the details of any complaints related to the scheme; and

-          declaration that the information submitted on the form and in any documents attached with, or submitted later as part of, the application are truthful and correct to the best of the applicant's knowledge and belief and that the applicant has signed the form willingly after reading the attached Applicant Information Sheet, Wildlife brochure and related materials.

 

All questions on the Certification Examination should be completed by the applicant, after reading the ESCS materials and the Instructions on the Certification Examination.  Each page of the Certification Examination should be signed by the applicant.  The Declaration Section of the Certification Examination must be signed in front of a witness and the witness must sign and complete their details.

 

The Anonymous Questionnaire will be separated from the application and sent directly to the DEWHA.  No identifying information about the applicant or their organisation will be recorded by AACMA or forwarded to the Department in relation to the Anonymous Questionnaire.

 

A non-refundable Application Fee of $165.00 (including GST) applies to each application and is payable with the application.  The Application Fee for AACMA members is waived.  A discounted Application Fee of $82.50 will apply to the first 200 applications received from non-members of AACMA, including traders and researchers. 

 

The above materials, when completed, should be forwarded with the Application Fee to the AACMA national office, and be addressed as follows:

When received, AACMA will process the application as follows:

-          undertake an initial document check to ensure all materials have been submitted;

-          issue a Tax Invoice for any payment;

-          forward a letter of acknowledgement with an application number;

-          undertake an assessment of your answers to the questions on the Certification Examination;

-          provide feedback and/or request re-submission of the Certification Examination if required;

-          determine eligibility according to the published criteria;

-          forward the Anonymous Questionnaire to DEWHA.

完成申请表上所有应填写的部分。请阅读申请表的声明部分并在一名见证人面前签名,并请见证人签名和填写其个人情况。

 

声明部分由以下几个成分组成:

-      授权AACMA在它认为是相关的,对随同申请时提交的第三方有关资料/文件作出查询,这包括但并不限于列于申请表上或在任何所附文件中的机构或团体;

-      同意遵守申请者须知概述的认证方案条款和条件;

-      同意接受信息和更新有关立法结构、环境保护和生物多样性保护法1999以及国际野生动植物合法贸易要求的知识;

-      声明本人或本公司只交易合法取得的野生动植物原料及加工品,本公司或本营业场所不提供非法交易取得的野生动植物药品及其他产品;

-      同意澳大利亚针灸中医药协会(AACMA)纪律委员会(或其继任者)受理与认证方案有关的投诉。

-      同意发放有关根据认证方案而签发的证书以及与认证方案有关的投诉情况的个人资料给环境,水利,遗产和艺术部;

-      声明表示尽我所知以及信仰,本表格提交的资料和申请所附的任何文件,或者以后提交的部分,是真实和正确的。在阅读所附申请者须知、野生动植物宣传手册和相关材料后,申请者已自愿在本表上签名。

 

阅读完濒危物种认证方案资料和认证考试指南后,申请人应完成全部考试题。申请人应在认证考试卷的每一页上签名。认证考试的声明部分必须在证人面前签名,证人也必须签名并填写其个人情况。

 

匿名问卷会与你的申请表分开,并直接发送给DEWHA。AACMA不会记录你或你团体的身份资料,也不会把它送往有关匿名问卷部门。

 

每一份申请需付申请费165.00澳元(包含商品及服务税),此申请费是不能退款的,并在提交申请时缴纳。首200份非AACMA会员申请(包括贸易商和研究人员在内)只需付折扣报名费82.50澳元。

AACMA收到申请后将按如下处理你的申请:

-      进行初步文件查核,以确保所有材料都已提;

-      签发税务发票;

-      发出收到申请的回执及申请号码;

-      批阅你的认证考试答卷;

-      提供反馈和/或要求重新提交认证考试,如果需要的话;

-      根据公布的标准,确定是否合格;

-      提交你的匿名问卷给DEWHA
